2021 Hand and Wrist Orthopedic Procedures with no SOSD

The attached spreadsheet, a much shortened version of the 2021 Medicare PFSRVU file, includes only 2021 Hand and Wrist Orthopedic Procedures with no SOSD in the following categories:

  1. Flexor And Extensor Tendon Repair
  2. Tendon Transfer
  3. Dupuytren Contracture Release
  4. Trigger Finger And Carpal Tunnel Release
  5. Surgical Treatment Of Metacarpal And Phalangeal Fractures.

These are the most common hand and wrist procedures performed under WALANT (Wide Awake Local Anesthesia with No Tourniquet). In total there are 97 procedures. If I missed any please let me know. I also have a longer PFSRVU SS on this website with over 1400 orthopedic codes with no SOSD.

If you plan on setting up an office-based surgical suite and submit claims with POS=11 then you will need to negotiate a fair reimbursement rate for any of the procedures on this list that you perform in the OBSS. Without the negotiated reimbursement you will only get the facility rate and zero compensation for your direct and indirect OBSS overhead costs.

The easiest negotiation tactic is simply ask the insurance company how much they are spending now on procedures performed at the ASC or HOPD under moderate sedation or general anesthesia and offer than a percent of that. Here are ASC, HOPD and hospital estimates for ECTR as an example. Start high expecting them to counter.
